3S/4S Sugarman Interdental Bone File, Buccal/Lingual is a double-ended interdental bone file, narrower and more targeted than a general-purpose bone file, built specifically for the tight confines of the interdental space during periodontal osseous surgery.
Its job is twofold: finishing and smoothing bone after a chisel or rongeur has already done the initial recontouring, and clearing granulomatous tissue from supra-bony and infra-bony pockets before flap closure. Both tasks call for a narrower, more precise instrument than the broader bone files used on facial or lingual surfaces, since the interdental space between adjacent roots leaves very little room to work.
The 3S/4S pattern carries a blade oriented for the cheek-side and tongue-side interproximal surfaces, completing the interdental access the 1S/2S pattern doesn’t reach, targeting the buccal and lingual surfaces of the interdental space specifically. This is used with a light filing stroke rather than a cutting motion, going over bone that’s already been roughly shaped to leave a smooth, physiologic contour that supports healthy tissue reattachment during healing, rather than a ledge or irregularity that would trap plaque or resist proper reattachment. Periodontists reach for this pattern specifically once the coarser chisel or rongeur work is done and the interdental contour needs finishing, a step that has a direct bearing on how cleanly the flap reattaches and how the tissue contour holds up over the long term.
