The 12 Miller surgical curette is the largest instrument in the Miller series, carrying 4.2 mm double-ended mirror-image spoon blades for debridement of wide molar and third molar sockets, large osseous defects, and cyst cavities where the 3.6 mm #11 spoon leaves insufficient blade-to-wall contact per stroke. The 4.2 mm blade covers the widest socket dimensions encountered in routine oral surgery within the Miller pattern, completing the series for cases that progress from narrow premolar to wide third molar extraction in a single surgical appointment. The 40° angled terminal shank and 22 mm reach apply to both ends. Medical-grade stainless steel, autoclavable.

The 12 Miller surgical curette completes the Miller series with its 4.2 mm spoon, the widest working end available in this pattern. At this blade width, the instrument is suited to the socket and defect dimensions encountered in third molar surgery, large cyst curettage procedures, and any extraction site where the combined socket dimensions at the working depth are wide enough that the 3.6 mm #11 spoon leaves too narrow a contact zone on the lateral socket walls to work efficiently.

Third molar extraction sites frequently present this requirement. The follicular tissue around an erupted or partially erupted third molar, and the pericoronal tissue around an impacted wisdom tooth whose crown was covered by soft tissue or bone, must be thoroughly curetted after the tooth is delivered. The dimensions of this follicular or pericoronal tissue zone often exceed 3.5 mm at the working width, particularly in the maxillary third molar region where the tuberosity area can provide a wide open socket after upper wisdom tooth delivery. The 4.2 mm blade covers this wider geometry in fewer working strokes, completing the debridement more efficiently and with lower risk of leaving residual follicular tissue or granulation tissue in the socket depth.

For large cyst debridement, the #12 is used alongside the #11 and in conjunction with Lucas #87 or #88 when the surgery involves both Miller and Lucas instruments. The 4.2 mm Miller spoon provides a different shank angle approach to the cyst cavity walls than the Lucas instruments, so the two series are often complementary rather than redundant in the same procedure.
