Castroviejo Needle Holder, Tungsten Carbide, 14cm

The Castroviejo Needle Holder is a fundamentally different instrument class compared to ring-handled needle holders. Its pencil-grip, spring-action design places it in the microsurgical instrument category, where tactile sensitivity and fine motor control take priority over the leverage and force capacity of a ring-handled instrument. Originally developed for ophthalmic surgery, the Castroviejo has become a standard instrument in periodontal microsurgery, mucogingival procedures, connective tissue grafting, and papilla reconstruction — procedures in dental surgery where suture sizes range from 5-0 to 8-0 and incision lines are measured in millimetres.

The Tungsten Carbide jaw inserts are particularly important in this design. Fine suture needles (6-0, 7-0) are small and delicate — a jaw surface without adequate grip will allow needle rotation or slippage that can collapse tissue margins and compromise wound edge adaptation. TC jaws provide a consistently firm, controlled bite on even the smallest needles used in intraoral microsurgery, with less jaw-closing pressure than standard SS alternatives.

The instrument is held between the thumb, index, and middle fingers in a pen or tripod grip. Squeezing the handles closes the jaws; releasing causes the spring to open them. This motion is much finer and more controllable than the gross motion of a ring-handled instrument, allowing the operator to make tiny, precise needle-driving movements with wrist and finger rotation rather than full forearm movement.

The locking clasp on the Castroviejo holds the jaws closed when the needle is set, allowing the operator to reposition fingers or release the instrument momentarily without losing needle position.

At 14 cm (5.5 inches), this instrument has sufficient overall length for both anterior and mid-arch intraoral access while remaining light and balanced enough for delicate microsurgical tissue handling.

Clinical Indications: Periodontal microsurgery, connective tissue grafting, papilla reconstruction, gingival flap closure with fine sutures, implant tissue closure using microsurgical technique, and frenectomy closure with fine suture materials.

Suture Compatibility: 5-0, 6-0, 7-0, and 8-0 sutures on small-radius curved needles used in microsurgical procedures.
