The defining feature is the double cutting edge on each end, set at 90 degrees to the lower shank rather than the 70-degree offset used on area-specific Gracey curettes. This gives the blade two working edges instead of one, so it can be used on facial, lingual, mesial, and distal surfaces without reorienting the handle.

Clinically, the 4R/4L is used for supragingival and subgingival scaling, root planing on anterior and premolar teeth, and general periodontal maintenance where a single versatile curette covers more of the arch than a specialized pattern.
