The Mayo Hegar Grooved Jaw Needle Holder 14cm is the everyday workhorse version of the grooved jaw series. The Mayo Hegar design is one of the most clinically familiar ring-handled needle holder frames in oral surgery and general dentistry – and this version upgrades the standard jaw with a longitudinal needle groove that directly addresses the most common technical issue in routine suturing: needle rotation under jaw-closing pressure.
When a curved suture needle is gripped in a plain cross-serrated jaw, it can shift or rotate slightly as the jaws close, particularly with smaller-radius or finer needles used in 4-0 and 5-0 suturing. Even a small degree of rotation changes the needle’s tissue entry angle, leading to inconsistent suture bite depth and uneven wound edge approximation. The center groove in this instrument’s jaw creates a dedicated channel – a needle track – that the needle body registers against when the jaw is closed. The groove prevents rotation mechanically, while the serrations on either side grip the needle laterally to prevent forward or backward slippage.
11.5cm vs. 14cm – Which size is right?
• 11.5cm: Anterior-only, pediatric patients, limited mouth opening – where a shorter instrument is clinically necessary
• 14cm: General-purpose – covers incisors through first premolars in adult patients; the recommended size for most routine dental surgical suturing
At 14 cm (5.5 inches), the instrument reaches comfortably into the premolar region of both arches without the handle pressing against the lip commissure during use. This makes it suitable for the broadest range of everyday intraoral suturing scenarios – post-extraction socket closure from canine to premolar, anterior implant site suturing, flap closure after crown lengthening, frenectomy, and soft tissue biopsy closure.
The box-lock joint keeps upper and lower jaws in precise alignment through repeated use – maintaining straight needle tracking through tissue. The ratchet lock provides multiple closing positions so jaw pressure can be matched to the needle and suture size in use. The standard ring-handle grip – thumb and ring finger in the rings, index finger along the shank – gives directional control during needle driving that most clinicians are already trained on.
Clinical Indications: Post-extraction socket closure (anterior to premolar zone), anterior and premolar implant site suturing, mucoperiosteal flap closure, frenectomy wound closure, soft tissue biopsy suturing, and any routine intraoral suturing in the anterior-to-premolar arch region.
Suture Compatibility: 3-0, 4-0, and 5-0 sutures on standard curved needles. The needle groove is particularly effective in controlling fine 4-0 and 5-0 needle rotation – the sizes most commonly used in dental soft tissue procedures.

Mayo Hegar Grooved Jaw vs. Standard Mayo Hegar
- Standard Mayo Hegar: Flat cross-serrated jaws – reliable grip but needle can rotate slightly under jaw pressure on fine curved needles
- Grooved Jaw Mayo Hegar: Center groove mechanically blocks needle rotation – same reliable Mayo Hegar frame, more consistent needle control
- Best upgrade for: Clinicians already using the Mayo Hegar who want improved needle orientation without switching to a TC instrument
